IDAM has provided a checklist for your work-related injury or illness
*This checklist applies to employees and interns paid by the lab. If you are an affiliate, please notify your
employer. If you are unsure, please contact Health Services.
- Injury Reporting:
- Report a possible work related injury or illness to your supervisor immediately.
- Contact Health Services for an evaluation. Health Services is open M-F 7:30-4PM.
- You may also see an external Occupational Medicine provider of your choice.

- Medical Treatment:
- Initial medical evaluation completed. Treatment plan established by medical provider.
- Follow-up medical appointments scheduled as needed. Appointments should be scheduled in a way that minimizes time off from work either by scheduling appointments before or after work hours.
- If your claim is approved and you need to be off work, the first 3 days off are considered a waiting period and are not paid by the insurance carrier (Sedgwick for CA claims, Liberty Mutual for out of state claims). You may use sick leave to cover this time period.
- If you receive Temporary Disability payments (TD), you must complete the Worker’s Compensation Election Form to indicate if you want to use accrued sick, vacation or both to supplement your TD payments which are two-thirdsof your salary. Additional information about payments.
- Claim Filing:
- DWC-1 Worker’s Compensation claim form provided to employee to complete and promptly return to Health Services.
- Claim form submitted to appropriate parties and claim number assigned by insurance carrier.
- Case Management: Our goal is to have you return to work safely and timely
- Health Services and IDAM will assist in coordinating medical care and any work restrictions.
- Employee is responsible to provide regular updates on their medical care and treatment progress.
- Injury and Illness Investigation:
- All work-related injuries, illnesses, and near misses (situations that could have led to injuries or illnesses) are important to report. We take these incidents seriously and will thoroughly review them following the Department of Energy guidelines. This helps us keep everyone safe at work.
- Return to Work:
- Forward work status reports including work restrictions, if any, IDAM for coordination
- of your return.
- Modified duty or accommodations explored.
- Return-to-work plan implemented.
- Monitoring of return-to-work progress.
- Claim Closure:
- Employee has reached maximum medical improvement.
