Lab employees who plan to work on site during the influenza season, November through April 30, 2024, must acknowledge they have either received an annual flu vaccination or decline to receive it by filling out an attestation form in the Occupational Health Record (OHR) system by Dec. 1. This complies with the UC vaccination policy. No proof of vaccination is required.
Represented employees should consult their collective bargaining agreement for information on their flu shot. If you receive your vaccination through the Lab’s Health Services, your attestation will be completed for you. Those who work remotely are encouraged to get a flu vaccine through their health care provider or a pharmacy.
To complete the attestation, go to clinic.lbl.gov>Message Center>Questionnarie-Flu Vaccination Declaration. Read the material and select the proper declaration before clicking on submit.
