If you work at a Lab site, you must acknowledge you are up-to-date with your COVID vaccination or decline to receive it by completing an attestation by Jan. 1 in compliance with the UC vaccination policy. No proof of vaccination is required.
Represented employees should consult their collective bargaining agreement for information on COVID vaccination. Those who work remotely are encouraged to get a COVID vaccine through their healthcare provider or a pharmacy.
To complete the attestation, go to clinic.lbl.gov>Message Center>COVID Vaccination Declaration Questionnaire 2023-24. Select the proper declaration before clicking on submit.
