IDAM has provided a checklist to help you prepare for your leave:
Preparing For Your Leave:
- As soon as you know that you need a leave of absence, notify:
- IDAM. Submit a request here or idam@lbl.gov
- Within 5 business days of notification, IDAM will review your eligibility and provide you with the necessary paperwork needed for your leave. If the leave must begin in less than 30 calendar days, the employee shall provide as much notice as possible

- *Please note: IDAM will not process leaves earlier than 30-45 days in advance. You can request a call with an IDAM team member in advance of your leave start date.
Processing Your Leave- What to Expect:
- If you are eligible, IDAM will provide you with the necessary paperwork to process your leave. (If you are not eligible, IDAM will provide you with your options)
- Return your paperwork to IDAM within15 calendar days of receipt
- IDAM will reach out to you to discuss your wage replacement options
- Within 5 business days of receiving all information, IDAM will enter your leave into UCPath. You will receive a designation letter with your time off dates and corresponding LETS codes
- If you are filing a disability claim through Lincoln Financial or The Standard (Post Docs only), you will need to initiate your claim before the start of your leave.

- If you will be on unpaid leave, contact Benefits at benefits@lbl.gov to make arrangements to continue your benefits and to pay for premiums during any unpaid portion of your leave. You can review your Benefits Summary, including cost, in UCPath.
- You can schedule a benefits consultation at https://calendly.com/lbnlbenefits/30min or you can call UCPath (855) 982-7284 to discuss your options to continue and pay for benefits while on an unpaid leave of absence. For paid leaves, your benefit deductions will continue as usual.
During Your Leave: (You are relieved of all work duties & responsibilities while on leave)
- Notify IDAM and your supervisor of any changes to your leave dates or ability to return to work.
- Your timekeeper and supervisor will assist with LETS entry and/or corrections.
- For maternity or parental leaves, you must notify idam@lbl.gov to provide an actual birth date, including the delivery method for maternity leaves, so updates to your pre-birth schedule can be made.
- You have 31 days from the birth of the baby to enroll your newborn into your benefits via UCPath at https://ucpath.universityofcalifornia.edu/home
- If you have questions, you can email benefits@lbl.gov, or you may contact the UCPATH center directly at 855-982-7284 (PST) Monday – Friday from 8:00 a.m. to 5:00 p.m.
Returning From Leave:
- Medical Leaves
- You must provide a Return to Work Certification or medical note releasing you back to work to IDAM on your first day back to work.
- Before your first day back at work, schedule a Return To Work appointment with Health Services (510-486-6266)
- All Leaves
- Notify IDAM if you have any issues accessing your computer systems
- System Access: FMS and LETS access may take up to 48 hours to restore. Contact the IDAM team with any questions or problems.
- Reach out to your Supervisor with any questions about your transition back to work
- Paycheck and Benefits Review: Upon return, review your paycheck and UCPath benefits status to verify accurate benefit premiums and enrollment. Contact UCPath (855) 982-7284 with any questions.
- UCRP Service Credit: Unpaid leave does not earn UCRP service credit. You may purchase service credit for the leave period (UCRP service credit buyback).
