If you’ve already entered your booster information at clinic.lbl, you do not need to read any further.
For everyone else, today is the deadline to upload your COVID booster card if you are medically eligible for a booster.
If you are not currently medically eligible, you must receive a COVID booster within 30 days of becoming medically eligible and upload proof of the booster.
If you have recently been diagnosed with COVID and you are out of your isolation window of 10 days after the diagnosis you can receive a COVID booster if your personal health provider agrees.
You can request an exception to the booster if:
- You are currently ill with COVID symptoms or have had COVID within the past 30 days
- You are requesting a NEW exception for any reason (medical, disability, religion, pregnancy)
If you have an approved exception for the initial vaccine series, it applies to the booster component as well. There is no need to submit an additional booster exception request in the OHR.
To request an exception, go to clinic.lbl.gov and:
- Click on COVID Booster Declaration>answer the questions about your first series of COVID vaccinations>answer have your received your 1st COVID booster “No”
- Select the unvaccinated reason, then follow instructions to complete the form Click submit. You will be contacted to discuss the next step in the exception process.
